Analysis of electrophysiological ECG/PPG recordings in a six-minute walk test in older adults

Abstract

In the modern world, the burden of cardiovascular diseases remains extremely high, ranking first among the causes of morbidity and mortality [Timmis A, et al. European Heart Journal 45.38 (2024): 4019-4062]. The problem of early diagnosis of the initial stages of cardiovascular disorders at the outpatient stage is very relevant. One of the methods that could be very useful in conducting such screening is the six-minute walk test (SMWT).
This paper discusses the possibility of expanding the standard evaluation criteria when conducting the SMWT (for example, the distance covered by the patient, the rate of recovery of the heart rate (HR), Borg scale, etc.). Wavelet analysis of the electrocardiogram (ECG) and photoplethysmogram (PPG) allows us to assess the effect of motor activity on weak changes in the rhythms of the cardiovascular system - both HR and vascular activity. For numerical analysis, the method of oscillatory wavelet patterns was used [Simonyan M, et al. The European Physical Journal Special Topics 232.5 (2023): 595-603]. The patient's step frequency during the active phase of the test is also detected based on the interference present in the recorded signals. The assessment of the ECG/PPG oscillatory components was compared with the patient’s clinical status and the dynamics of their disease, which formed the basis for additional criteria for a detailed assessment of the state of the cardiovascular system.
